Case Management - Adult
If you or a loved one has serious and persistent mental illness or a dual diagnosis, case management comprises a system of supports and services that can help. Our compassionate, experienced team is equipped to help address the barriers between you and a good quality of life. We work with you to provide planning, referrals and advocacy and can provide assistance with transportation, medical and psychiatric care, legal issues and basic living needs such as housing and food. Traditional Level Service
Traditional Case Management – This level of case management provides services to help you successfully control your illnesses and maintain the supports and skills necessary for living in the community. Supportive guidance is provided.
Intensive Level Service
Florida Assertive Community Treatment (FACT) – FACT uses a “hospital without walls” approach if you have a serious mental illness and are at risk because of the severity of your symptoms, low functioning or co-occurring substance abuse. Our FACT Team comprises professionals in psychiatry, nursing, counseling, case management, substance abuse treatments and vocational services. We collaborate to ensure that you receive a complete set of supports and services that will allow you to safely remain in the community.
Community Recovery Team (CRT) – CRT is our most intensive level of care within case management. Our team is dedicated to working with you through the transition from acute care or a state mental health hospital, helping you adjust to any challenges you may discover in community living.
