We want to hear from you! Our team wants to ensure we are meeting the needs of the communities we’re privileged to serve.  Please take a few minutes to complete the survey if you are 18 or older and fit one or more of the categories below:

• Resident of Escambia, Santa Rosa, Okaloosa, or Walton County, Florida
• Person who receives or has received services from Lakeview Center
• Family member of a person who receives or has received services from Lakeview Center
• Person who receives or has received behavioral healthcare services elsewhere
• Healthcare or behavioral healthcare service provider in Escambia, Santa Rosa, Okaloosa, or Walton County, Florida

The goal is to learn more about the healthcare and behavioral healthcare needs of the entire community, including those receiving and not receiving services they need.  The survey focuses on  healthcare and behavioral healthcare service needs and barriers.  It should take most people less than 10 minutes to complete. 

The survey is available online.  Your participation is voluntary. You may skip any questions or complete only part of the survey.  All survey responses will remain completely anonymous and no one will ever know anyone’s individual answers. 

Names and contact information will never be connected to the survey responses and will be deleted after the drawing is complete.
