Lakeview Center of Walton County – formerly Chautauqua Healthcare Services – works to strengthen families and make a positive impact from our location in DeFuniak Springs. The team provides counseling to a variety of people experiencing a wide range of mental health and substance misuse issues.

Our services

Adults and children can access a comprehensive array of mental health and substance misuse treatments including:

  • Outpatient counseling including anger management
  • Psychiatric treatment
  • Crisis counseling
  • Case management
  • Screening and referrals to services
  • Outreach for individuals struggling with homelessness and mental health issues
  • Residential treatment for women struggling with substance misuse
  • Residential treatment for adults with chronic mental illness who need help learning daily living tasks like paying bills and taking medications. Lakeview Center caregivers teach skills to help prepare those we serve go back into community again.

For people who are referred to us for care by court order, Lakeview Center provides:

  • Anger management
  • Parenting group
  • Batterer’s intervention groups for men and women

Bringing care to you

Our Community Action Team (CAT) serves children in homes, schools and daycare settings to help lower the risk of hospitalization, reduce contact with law enforcement and improve functioning within the home, school and community settings. The goal is to help the child/adolescent and family become successful in the environment where they live, preventing the need for residential treatment.

In Walton County, CAT serves youth ages 11 to 21.

Crisis care

The Lakeview Center Mobile Response Team (MRT) serves Escambia, Santa Rosa, Okaloosa and Walton counties. MRT provides 24/7 intervention and support and is available to respond within 60 minutes via telephone triage or in-person. We can come to you. In addition to helping individuals, we partner with local law enforcement, schools, emergency rooms and other providers. MRT also provides after-care to ensure a smooth transition to the appropriate support services.

JoAnn Birge Women and Children’s Center

Located in DeFuniak Springs near Lakeview Center of Walton County, this center is a residential treatment program for women struggling with substance misuse. Our team helps women, primarily those who are pregnant or postpartum. Mother and child can remain together during this crucial time.

Family support team

The goal of our family support team is to help families remain together as long as the children are safe in their homes. This unique Walton County program provides wraparound services and in-home supportive services to children in non-judicial cases. The family care unit provides assessments for parents involved in a judicial dependency case. All families are referred by the Department of Children and Families and FamiliesFirst Network.

Psychosocial rehabilitation

Lakeview Center of Walton County also provides daily psychosocial rehabilitation. The goal is to restore the skills and abilities necessary for independent living by promoting:

  • Development and maintenance of daily living skills
  • Food planning and preparation
  • Money management
  • Maintaining a living environment
  • Appropriate use of community services

The program helps individuals who have a mental health diagnosis and require medically necessary behavioral health community support services, while exhibiting one of the following symptoms severe enough to cause significant impairment in daily functioning:

  • Psychiatric
  • Behavioral or cognitive
  • Addictive behavior
  • Clinical conditions

PATH

Projects for Assistance in Transition from Homelessness (PATH) is designed to help people struggling with mental health issues to obtain housing and stability.

PATH links eligible people to mental health and substance abuse services in Okaloosa and Walton counties.

Case managers can also provide referrals to our community partners for services such as:

  • Shelter
  • Transportation
  • Habilitation and rehabilitation
  • Employment
  • Permanent housing
